Most operational problems do not show up as "system issues". They show up as small, annoying losses that everyone accepts as normal.
I watched this play out recently in a manufacturing business.
The office prepared documents and schedules. The loading team worked from handwritten lists. Someone thought one unit was already on the truck. It was not.
Nothing dramatic happened. No shouting. No crisis.
Just phone calls. Clarifications. Rechecking paperwork. Walking back and forth. Fixing something that should have been clear from the start.
By the end of the day, everyone had done their job. And half a day was gone.
This is not a people problem. It is what happens when information lives in too many places and no one owns the current version of reality.
Most teams only notice this pattern when projects start overlapping and pressure increases. By then, the cost is already baked in.
